Cycle notes

Bring dates for recent periods, cycle length if you know it, missed periods, heavy bleeding, spotting, pain, and any major change from your normal pattern.

Symptom notes

Write down symptoms you are tracking and how often they show up. Examples include acne, hair growth changes, hair thinning, fatigue, bloating, cravings, mood shifts, sleep changes, headaches, cramps, or digestive symptoms.

Medication and supplement list

Bring the names, doses, and timing for medications, vitamins, herbs, and supplements. This matters because supplements and prescriptions can interact, and your clinician needs the full picture.

Labs and questions to ask

What labs or follow-up testing make sense for my symptoms and history?
Could insulin resistance, thyroid issues, androgen levels, or other factors be part of this pattern?
What treatment options fit my goals, such as cycle regularity, acne, hair changes, fertility, cravings, or weight support?
Are any medications or supplements unsafe for me?
